New framework for chronic disease prevention and care
The province is launching a new framework to guide efforts to reduce the impact of chronic disease in Prince Edward Island and help Islanders live longer, healthier lives.
The Chronic Disease Framework is a province-wide approach that brings together government, healthcare and community partners, and Islanders to help people stay healthy, prevent chronic disease where possible and improve coordinated care for those living with chronic disease. It will guide future policies, strategies, and services through three pillars: prevention; screening and early detection; and disease management.

We want Islanders to have the best possible health and quality of life, and we want those living with chronic diseases to have the support and resources they need to manage their health. This framework gives us a roadmap to do that work together."

Chronic diseases account for seven of the 10 leading causes of death in Prince Edward Island and Canada. Many chronic diseases have shared risk factors, and evidence-based management and coordinated care can improve prevention, early detection, and long-term health outcomes.
“This framework reinforces our commitment to providing person-centred care that supports Islanders at every stage of chronic disease prevention and management," said Health PEI interim Chief Executive Officer Laurae Kloschinsky. "By strengthening coordination across healthcare services and working closely with community partners, we can help Islanders access the right care and support at the right time and make it easier to navigate the services they need.”
The Chronic Disease Framework supports government's commitment to building a stronger, more sustainable healthcare system. It also aligns with Live Well PEI and the Provincial Health Plan 2023–2028.
“The Live Well PEI approach recognizes the many factors that influence health and the importance of reducing barriers to healthy living,” said Chief Public Health Officer Dr. Heather Morrison. “The Chronic Disease Framework builds on this work, creating a shared focus across communities and the healthcare system. Together, we can help prevent chronic disease and support healthier futures for all Islanders.”
Developed with input from Island residents and key partners, the Chronic Disease Framework will guide future initiatives under each pillar. Continued engagement, leadership, and collaboration will support implementation in the years ahead.
